Honorariums
What better gift to give than to honor a living person with a gift to Community Hospital Health Foundation. Honor family and friends on their birthdays, anniversaries, and special holidays.

Memorials and honorariums are the gifts that keep on giving. All charitable gifts to Community Hospital Health Foundation go directly towards helping provide advanced care, always there through Community Hospital.

To recognize each gift, a card will be sent to the family or the honored person to notify them of the thoughtfulness expressed by you, the donor. The amount of the gift will not be disclosed. Each quarter the gifts are listed in the quarterly publication sent out by Community Hospital. The gifts will also be listed on the Community Hospital Health Foundation website.

This gift form is provided so that you may give your gift that will touch the lives of countless others.
